Pre-op care includes screening for mental health issues, OSA, cardiovascular/VTE risk, and micronutrient deficiencies; mandated pre-op weight loss is not evidence-based. Key procedure-specific issues include GERD after sleeve, ulcer risk with bypass (worse with smoking/NSAIDs), and lifelong micronutrient monitoring. Weight regain can occur; anti-obesity medications may help when response is inadequate or regain develops.
