Dr. Dipna Gallagher discussed how body composition measurement must be matched to the research or clinical question, because no single method is best for every situation. She emphasized that many commonly used techniques rely on assumptions that can be invalidated by age, growth, disease, obesity, weight loss, or especially changes in hydration. For example, weight stability does not mean body composition is stable, and weight loss does not necessarily mean proportional fat loss.<br /><br />She reviewed major measurement tools, including anthropometry, skinfolds, bioelectrical impedance analysis, air displacement plethysmography, dilution methods, DEXA, quantitative magnetic resonance, MRI, CT, and doubly labeled water for energy expenditure. She highlighted the strengths and limitations of each, noting that DEXA is useful for bone and regional lean mass but has important limitations in the trunk and in people with obesity. MRI and CT provide more detailed information about visceral fat, intramuscular fat, and skeletal muscle quality.<br /><br />Dr. Gallagher also showed how body composition is highly relevant in clinical conditions such as AIDS wasting, cancer, osteoarthritis, mitochondrial disease, aging, and bariatric surgery. Across these examples, loss of muscle quality and muscle mass often predicted worse outcomes more strongly than body weight or BMI alone. She stressed that strength can decline faster than muscle mass, so function matters as much as quantity.<br /><br />Finally, she discussed weight-loss maintenance research and noted that newer anti-obesity medications like GLP-1s may alter hydration and therefore complicate body composition interpretation. Her main message was that accurate body composition assessment requires careful method selection, awareness of assumptions, and consideration of the population and intervention being studied.
