Dr. Elizabeth Parks presented research on metabolic-associated steatotic liver disease (MASLD) and how obesity, diet, and exercise influence liver fat. She explained that liver fat can come from dietary fat, fatty acids released from adipose tissue, and especially de novo lipogenesis (DNL), the liver’s conversion of sugars into fat. Her studies showed that eating, particularly carbohydrate-rich meals, strongly stimulates DNL, and that some people are more susceptible than others.<br /><br />In a “tailgate” overfeeding study, participants ate and drank heavily for one afternoon. Some developed marked increases in liver fat, while others did not. The biggest predictor of worsening liver fat was increased DNL, suggesting that sugar-driven fat production is a major driver of fatty liver.<br /><br />She then described a weight-loss study showing that reducing energy intake lowered liver fat, improved insulin sensitivity, and reduced DNL, especially in people who began with fatty liver.<br /><br />Finally, she presented a 10-month diet-and-exercise trial in patients with more severe disease (MASH), where high-intensity interval training plus dietary counseling improved liver histology, fitness, fibrosis, and liver fat. Her overall message: reducing nutrient overload, especially late-day excess calories and added sugars, plus regular exercise, can meaningfully improve liver health.
