Video Summary
Dr. Luca Basusto presented a Grand Rounds arguing that obesity should be assessed and treated as a chronic, progressive, relapsing disease. He emphasized that diagnosis should not rely on BMI alone, but on both abnormal fat accumulation and the health consequences of that fat, especially fat distribution and function. He discussed the limitations of BMI, noting that it can miss high-risk patients with normal BMI but high waist-to-height ratio, as well as patients with sarcopenic obesity.

He proposed using staging systems, such as the Edmonton Obesity Staging System, to better capture medical, mental, and functional severity and to track disease progression and treatment response. He also challenged the field’s reliance on percent weight loss as the main treatment target, arguing that final BMI and waist-to-height ratio after treatment are more useful predictors of long-term complication risk.

Using large UK database studies, he showed that BMI and waist-to-height ratio predict different complications differently: BMI matters more for osteoarthritis, while waist-to-height ratio better predicts diabetes and cardiovascular disease. He concluded that obesity care should become more individualized and prescriptive, with treatment intensity matched to disease severity and likelihood of reaching meaningful risk-reduction targets.
